Medicaid Applications – Allow us to handle all the details – from collecting the required information and documentation, to making the actual application, to following through with the appropriate agencies until final Medicaid approval is achieved.
Care Management - Following an individualized assessment of a person's needs and goals, we create a Care Plan, which is like a road map, whereby we address all of the issues that were noted in the assessment, and we work to bring solutions and positive outcomes.
Home Care – We work with you, and your loved one, to understand what type of assistance is needed to create a positive care and home situation. Understanding the unique needs of each client, and their family, and how to successfully match caregivers to those needs is at the heart of our success.
Elder Care Planning – Proper planning will ensure the effective management of your personal, healthcare and financial affairs for when you may not be in a position to do so. We provide expert advice and guidance to help you develop an Elder Life Plan.
Crisis Management – We’re available 24 hours a day, seven days a week for assistance in any type of crisis situations, including acute hospitalization, medical trauma or a sudden change in the living situation.
Psychotherapy Counseling – Counseling can be very beneficial in dealing with concerns about life changes, illness, losses, feelings of isolation, sadness or depression, loneliness, difficulty in getting along with others, or just an identified need for "someone to talk to." We bill Medicare Part B and accept Medicare assignment for our services.
Legal Appointments – Includes Financial Power of Attorney, Medical Power of Attorney, Conservator and Guardianship appointments.
Medical Claims Management – Working directly with medical providers, pharmacies and insurance companies to effectively manage claims and benefits.
Account Management – Offering assistance either in the client's home, or our office, with bill pay and other financial management issues.
